Tubal Surgery to Unblock Fallopian Tubes (Tuboplasty)
Fallopian tubes blockage is due to various reasons. If fallopian tubes are blocked at the distal end (away from the womb) and is usually due to tubal or pelvic infection, adhesions or endometriosis. In distal end blockage TCTC is not a suitable method to clear the blockage. Surgery under general anaesthetic, open abdomen is indicated. In these cases, the distal opening of the tube is reconstructed and stitched in such a manner to prevent closure again. Adhesions are normally divided as much as possible. Ovaries might be burried in adhesions but normally cleared during the operation.
After repairing the tubes, a dye called Methylene Blue is injected via the cervix to ensure the success of the operation. Any other ovarian problem could be dealt with at the same time. This procedure carries high success rate and patient may stay one night in hospital or go home the same day. We usually use a medicine to prevent the formation of adhesions after surgery.
We normally will see the patient on week after surgery and a month later when blood may be taken for hormones testing. A month later fertility drug may be given to speed up the desired conception.
Unfortunately, the number of the experienced surgeons performing this type of surgery is declining as most of the new generations are trained for IVF rather than this delicate type of surgery. But once again, patients should know all the options available to them and they have the right to choose.
